Our pharmacy practice serves Patients and does not serve at the request of a dispensing pharmacy, PBM, nor an insurance company. Our foundation is the Patient-Centered Primary Care Medical Neighborhood Model Concept as proposed by CMS/HHS. We encourage health insurance brokers to seek us out to engage a health and wellness aspect to their programs, including pharmaceutical care, and Pharmacogenomic evaluation to prevent ADR. We also welcome Federally qualified primary care clinics seeking to engage and to utilize the medical home model, based on the Social Determinants of Health, to contact us and review our model - which demonstrates solid outcomes for standards of care and positive patient outcomes for chronic disease management.
